Arteria Carotis Externa'nin Lateral Pozisyonu

Abstract

Arteria carotis externa cartilago thyroidea'nın üst kenarı düzeyinde başlar, hafif bir kavis çizerek yukarı öne doğru ilerler. Arteria carotis externa ve Arteria carotis interna'nın bifurkasyon noktası ile ilgili çok sayıda varyasyon ve anomalilerin olduğu bilinmektedir. Arteria carotis externa'nın lateral pozisyonunun da önceden tanımlandığı gibi nadir olmadığı bildirilmektedir. Dokuz Eylül Üniversitesi Tıp Fakültesi Anatomi Anabilim Dalı laboratuvarında formaldehit ile fikse edilmiş bir erkek kadavrada sağ arteria carotis externa'nın arteria carotis interna'ya göre lateral yerleşim gösterdiği belirlendi. Olguda bifurcatio carotidis, arteria carotis externave dallarının lokalizasyonu, bifurcatio carotidis ile nervus hypoglossus ilişkisi anatomik landmarklara göre değerlendirildi. Değerlendirme için cornu majus'un arka alt ucundan, glandula thyroidea'nın loblarının üst kenarından, ve prominentia laryngea'dan geçen horizontal düzlemler anatomik landmarlar olarak belirlendi. Literatür bilgilerine göre bazı hastalar arteria carotis externa'nın lateral pozisyonu nedeniyle nervus hypogossus basısına bağlı olarak gelişen semptomlarla hekimlere başvurabilmektedir. Bu varyasyonun hastalığın tanısında akla getirilmesi ve bifurcatio carotidis yerleşim bölgesine yapılan cerrahi yaklaşımlarda göznünde bulundurulması önem taşımaktadır. The external carotid artery begins opposite to the upper border of the thyroid cartilage and taking a slightly curved course, passes upwardly and forwardly. It is known that there are a number of variation and anomalies in the bifurcation of the external carotid artery and internal carotid artery. It has also been reported that the lateral position of the external carotid artery, which was formerly condidered exteremely rare, is not so unusual. During the dissections of formalin fixed adult male cadaver, we observed that the lateral position of the external carotid artery in his right neck region in anatomy department of Dokuz Eylül University Medical Faculty. The location of the carotid bifurcation, external carotid artery and its branches and the relationship between the bifurcatio carotidis and hypoglossal nerve was evaluated according to anatomic landmarks. For the evaluation, the determined anatomic landmarks were the horizontal planes passing from the postero-inferior end of the great horn upper border of the thyroid gland and the laryngeal prominence. In literature, some patients admit to the clinicans due to sympthoms related to the impression of lateral position of arteria carotis externa on nervus hypoglossus. This variation should be remembered in diagnosis of the illness and care must be taken in the surgical approaches to the localization area of bifurcatio carotidis.
