Abstract:
GİRİŞ ve AMAÇ Travma vakalarında, kontrol edilemeyen kanamalar, hastane öncesi dönemde başta gelen ölüm nedenidir ve yaralanma sonrası etkin kanama kontrolü, mortalite ve morbiditeyi ciddi oranda azaltabilir. Efektif kanama kontrolü, savaşlar, afetler gibi yaralı sayısının fazla, medikal gücün sınırlı olduğu ortamlarda ve özellikle kanama eğilimini arttıran heparin gibi medikal tedavi alan hastalarda önemli bir problem oluşturur. Bu çalışmada, microporous polysaccharide hemosphere'in (MPH) heparinize ratlarda hemostaz süresine etkisini araştırmayı amaçladık. MATERYAL VE METOD Çalışmamızda, 10 adet wistar albino suşu dişi ratı heparinize ettikten sonra femoral arterlerinde 22 gauge branül ucu ile delik oluşturduk. Kanama alanına, bir grupta sadece standart ağırlıkta bası, diğer gruba ise standart ağırlıkta bası ve MPH'ı (TraumaDEX®) birlikte uyguladık. İki grup arasında kanama zamanı açısından fark olup olmadığını araştırdık. BULGULAR MPH uygulanan grupta beş rattan dördünde kanama 60.sn'de, bir tanesinde 90.sn' de durdu. Kontrol grubunda hiçbir ratta 90.sn'de bile hemostaz sağlanamadı. İki grup arasındaki fark istatiksel olarak anlamlıydı (p=.001). TARTIŞMA VE SONUÇ Femoral arter kanamalı heparinize rat modelinde, standart bası ile beraber MPH uygulanılması hemostaz süresinde anlamlı bir azalma sağlamıştır. OBJECTIVES Uncontrolled bleeding is the major cause of death in trauma patients in prehospital setting and effective bleeding control after injury will reduce the mortality and morbidity. Bleeding control is an important problem in cases like war and disaster in which there are a lot of injuried patients with less medical equipment, and especially in patients who use drugs like heparine that may increase bleeding. In this study, we intended to find the effect of microporous polysaccharide hemosphere (MPH) on hemostasis time in heparinized rats. METHOD In our study, 10 wistar albino adult female rats were heparinized, then the femoral artery of each rat was pierced by a 22 gauge cathater to initiate bleeding. We placed standart scale weight on bleeding area in control group and standart scale weight and MPH in study group. We analyzed the difference between bleeding times in two groups. RESULTS Application of MPH resulted in complete control of bleeding in 4 of 5 and 1 of 5 rats at 60 and 90 seconds, respectively. In the control group, however, hemostasis could not be achieved in all five rats, even at 90 seconds. The difference between the two groups was statistically significant (p=.001). CONCLUSION Application of MPH and compression with a scale weight significantly decreased the time of hemostasis in a heparinized rat model with femoral arterial bleeding.
