Dokuz Eylül Üniversitesi Uygulama ve Araştırma Hastanesi bildirimi zorunlu bulaşıcı hastalık bildirimlerinin değerlendirilmesi (2005-2008)

Abstract

Bir ülke veya bir bölgede bulaşıcı hastalıkların türleri ve sayısı ile ilgili verileri elde etmek bu hastalıkların önlenmesi ile ilgili yapılan çalışmaların planlanmasında önemli yere sahiptir. Bu tanımlayıcı çalışmanın amacı Dokuz Eylül Üniversitesi Uygulama ve Araştırma Hastanesi'nden bildirilen Bildirimi Zorunlu Bulaşıcı Hastalıkların özelliklerini belirlemek, kayıtlardaki eksiklikleri saptamak ve 2008 yılında eksik hastalık bildirimi olup olmadığını bazı hastalıklar özelinde değerlendirilmektir. Ocak 2005 - Aralık 2008 tarihleri arasındaki son dört yıl içinde yapılan tüm bulaşıcı hastalık bildirimleri bildirim formları üzerinden değerlendirildi. Ayrıca 2008 yılında, belirlenen ICD-10 kodları üzerinden, bildirilmesi gereken hastalıklar çıkarılarak bildirilenlerle karşılaştırıldı. Son dört yıl içinde belirlenen 1199 geçerli bildirimin %56.6'sı erkeklerden oluşurken bu hastaların %42.3'ü 15-49 yaş grubu hastalardan oluşmaktadır. Bildirilen hastalıklar arasında A grubu hastalıklar ilk sırada görülürken genel olarak Kuduz ve Kuduz Riskli Temas, Tüberküloz ve Salmonella en sık bildirilen hastalıklardır. Bu hastalıkların bildirimi en fazla Acil Tıp Anabilim Dalı'ndan yapılmıştır. Bildirim formlarında en fazla eksiklik hastaların meslek bilgisinde olduğu görülmüştür. 2008 yılı içinde bildirilmesi gereken hastalıkların ne kadarının bildirildiği incelenmiş ve kodlama yanlışlıklarının olduğu gözlenmiştir. Tüberküloz açısından incelendiğinde tanı alan hastaların %25.0'nin bildirilmediği belirlenirken, bildirilen 51 olgunun 24'üne uygun ICD-10 kodu verilmediği saptanmıştır. Sonuç olarak kayıtlarda Bildirimi Zorunlu Bulaşıcı Hastalıklar yönünden eksikler ve hatalar gözlenmiştir. Bildirimi Zorunlu Bulaşıcı Hastalıkların bildirimi ve kodlanması ile ilgili eğitimlerin düzenli olarak sürdürülmesi gerekmektedir. To obtain data on communicable diseases in a country or a region has an important place in the planning of activities for the prevention and control of the diseases. The aim of this descriptive study is to determine the characteristics of diseases notified between 2005 and 2008, to evaluate the lack of information in the notification forms, and to assess the completeness of notification in 2008. The characteristics of diseases notified in the last four years were evaluated using notification forms. Also in 2008, the completeness of notification was assessed by comparing the diseases notified with the diseases required notification, identified by ICD-10 codes. Between 2005-2008, 1199 communicable diseases were notifiedæ 56.6% of the cases were male, 42.3% were in the age group of 15-49. Group A diseases and, in general, rabies and rabies risky contact, tuberculosis and salmonellosis were the most frequently notified. Most of the notifications were made by the department of emergency medicine. It was observed that there was great lack of information about the occupation of the patients in the notification forms. In 2008, the diseases required notification were identified by ICD-10 codesæ coding errors and also incompleteness of notification were observed. It was determined that 25.0% of tuberculosis cases were not notified and 24 of notified tuberculosis cases were not accurately coded. In conclusion, there are some errors in the notification and coding of the communicable diseases. There is a need for the appropriate and continuing training of health workers on these subjects.
